- New or acute diagnosis
- Medication change or medication non-compliance issues
- Changes in existing medical conditions
- Situations requiring assistance or teaching from a nurse or therapist
- Blood pressure and blood sugar monitoring
- Need for home care assistance following discharge from hospital, nursing home, rehab facility, emergency room, after a visit to an outpatients facility or physician’s office
- Wound care (Including wound vacuums)
- Instruction in nebulizer treatments
- Diabetic care, administration of insulin, and teaching
